A unique tool to guide the industry along the path to maximally affordable, highest quality healthcare in the US & beyond By EW Sharing riveting examples from his role as Director of Managed care for one of California’s top medical groups, Shapiro illustrates a robust process through which the health industry can maximize quality and affordability of care for patients. Highlighting pitfalls and barriers, Shapiro also details those strategies that he and his team have found most useful to surmount them. This work carries unique insight - information found nowhere else. Herein lies guidance we cannot do without as we navigate the system-wide transformation upon us, with the shared goal to minimize human suffering, reduce the burden of disease and achieve sustainable care, accessible to all.

By CnT I had the great pleasure of reading a pre-release copy of Dr. Shapiro's book - what an amazingly important time for this information!Shapiro is clearly an “Agent of Change” for health, and at a time most needed. Here, he describes Variation Reduction, an innovative process guiding physicians in the evaluation of their own practice data to maximize quality and affordability of care. Shapiro illustrates a generalizable, patient-centered, collaborative platform to identify and maintain value in any health care delivery system. The book shares the fascinating background for his vision for the program, including his unique insight into the varied and conflicting economic, legislative, ethical and emotional forces affecting the range of stakeholders in the US health care market, including care providers, insurers, patients and employers. Shapiro’s illumination of the recent history and current context, along with his presentation of the Variation Reduction model as a clear and transferrable process, struck me with a great sense of relief. I see the relevance of this model in a variety of settings, including both national and international low resource settings – although data may be increasingly available in many such settings, few understand how to use information quickly and effectively to target resources to working solutions, and therein effectively reduce the burden of disease. The power of information is astonishing, and Shapiro's strategy is highly applicable. It is exciting to consider how powerful this process can be, even in the face of the crushing burden of disease in so many areas. Tools like Variation Reduction are widely needed, and may well emerge as a key to effective, affordable health care on a worldwide scale

By Grala96 Kudos to Dr Lawrence Shapiro and the leadership at Palo Alto Medical Foundation for addressing clinical variation and cost of care. What a refreshing change to see physicians taking responsibility for their role in healthcare costs at a grassroots level. Dr Shapiro provides an excellent overview of the history of clinical variation and gives practical information on how to start a variation reduction program in your own institution. A must read for physicians interested in providing appropriate, value driven care for their patients.
